FIRST DAY EXCLUSION - REQUIRED PHYSICALS/IMMUNIZATIONS MUST BE ON FILE TO START SCHOOL

All students beginning preschool, Kindergarten, 6^th and 9^th grades in FSD145 are required to provide proof of a current State of Illinois physical examination and various required vaccinations by the first day of school in August 2024. Students in 12th grade must also provide proof of certain vaccinations. Students new to FSD from outside the state must also have a current physical, regardless of grade. Please be advised that students will not be admitted on the first day of school if they don’t have the required physical and immunizations on file. Therefore, please arrange for any required physicals and immunizations for your student with a medical provider as soon as possible. DENTAL & VISION EXAMS

The State of Illinois requires all students in Kindergarten, 2^nd and 6^th grades to provide proof of a dental examination to the school by May 15, 2025. All Illinois children upon first entry into school, including all Kindergarten students, must also provide proof of an eye exam  performed by an ophthalmologist or an optometrist by October 15, 20224
