• In an effort to ensure the health and safety of all students and to ensure compliance with state law and Illinois School Code, Freeport School District 145 has established “First Day Exclusion”. Under Board Policy 7:100, students will not be allowed in school if they don’t have the required Illinois physical and current immunizations on file by the first day of school. If you are the parent or guardian of a Freeport School District student, please arrange for any required physicals and immunizations with a medical provider as soon as possible.

    Requirements by Grade

    Preschool

    • Physical Exam by a Primary Care Provider
    • Immunizations: measles, mumps, rubella (MMR); tetanus, diphtheria, pertussis (DTaP); polio (IPV); hepatitis B (HepB); haemophilus influenza type B (HIB); pneumococcal (PCV)

    Kindergarten

    • Physical Exam by a Primary Care Provider
    • Immunizations: measles, mumps, rubella (MMR); tetanus, diphtheria, pertussis (DTaP); polio (IPV); hepatitis B (HepB); haemophilus influenza type B (HIB); pneumococcal (PCV)
    • Dental Exam (by May 15 of the current school year)
    • Vision Exam (by October 15 of the current school year)

    Grade 2

    • Dental Exam (by May 15 of the current school year)

    Grade 6

    • Physical Exam by a Primary Care Provider
    • Immunizations: measles, mumps, rubella (MMR); tetanus, diphtheria, pertussis (Tdap); polio (IPV); hepatitis B (HepB); varicella (VAR, chicken pox); meningococcal conjugative (MCV)
    • Dental Exam (by May 15 of the current school year)

    Grade 9

    • Physical Exam by a Primary Care Provider

    • Dental Examination

    Grade 12

    • Immunizations: measles, mumps, rubella (MMR); tetanus, diphtheria, pertussis (Tdap); polio (IPV); hepatitis B (HepB); varicella (VAR, chicken pox); meningococcal conjugative (MCV) after 16th birthday.

    Students enrolling in an Illinois school for the first time (regardless of grade)

    • Physical Exam by a Primary Care Provider
    • Vision Exam (by October of the current school year)
    • Dental Exam (by May 15 of the current school year)
    • Up-to-date immunizations

    Exemptions

    Religious: Parent/legal guardian and primary care provider must complete Illinois Certification of Religious Exemption to Required Immunizations and/or Examinations and return to school nurse by First Day of School of the school year.

    Medical: Letter from primary care provider stating reason for medical objection for immunizations and attach the letter to State of Illinois Certificate of Child Health Examination.
