Physicals and Immunizations
All children entering preschool, kindergarten, sixth grade, ninth grade, and twelfth grade and children who are newly-enrolled in District 145 are required by law to have a complete health examination by a licensed physician one year before beginning school. Immunizations against communicable diseases are required and strictly enforced by the Illinois Department of Public Health. Freeport School District School Board policy requires that information regarding these medical procedures be presented to the nurse before the first day of school.
Physicals can be obtained from your family doctor, nurse practitioner or physicians assistant working with a licensed medical doctor.
